Support, Incorporated provides the following services:

 

Diagnostic Assessment, Evaluation and Therapy Services, is a mental health provider, which offers a range of clinical services for children, their families, and adults.  Therapeutic services include individual outpatient therapy, family therapy, group therapy, screenings, psychological testing, assessments/evaluations and case management.

 

Our Staff:

 

Ben Johnson, Psy.D., is a licensed clinical psychologist in the state of North Carolina and is the Clinical Director of Support, Inc.  Dr. Johnson received his B.A. degree at the University of North Carolina at Chapel Hill and his Doctorate in Clinical Psychology from California School of Professional Psychology.  Dr. Johnson has extensive experience working with children, adolescents and adults within a variety of settings.  He has provided therapeutic services for adolescent gang members in Fresno, California, as well as severely emotionally disturbed children residing in Sacramento California.  He also provided both long and short-term therapy with adults, focusing on variety of issues ranging from interpersonal relationships and personal growth to health issues, depression and anxiety.  Dr. Johnson served on the faculty at Alliant International University (AIU), where he taught the Intellectual Assessment course for students working towards their doctorate degrees.

Shavonda Green, MA, LPA (Licensed Psychological Associate) received an undergraduate degree in Psychology from Winston-Salem State University and a Graduate degree in Clinical Psychology from Appalachian State University. Ms. Green is a board certified Health Service Provider in the state of North Carolina. She provides individual, group, and family therapy, as well as psychological evaluations to assess personality in intellectual functioning. She has seven years of experience providing therapeutic services to children and their families. Play therapy and cognitive-behavioral techniques are primarily used to assist individuals in reaching their personal treatment goals.

Case Management Services

Case management (MH/SA TCM) is an activity that assists recipients to gain access to necessary care: medical, behavioral, social, and other services appropriate to their needs. Case management is individualized, person centered, empowering, comprehensive, strengths-based, and outcome focused.

 The functions of case management include:

  • Case Management Assessment- A comprehensive and culturally appropriate case management assessment documents a recipient’s service needs, strengths, resources, preferences, and goals to develop a Person Centered Plan.

  • Person Centered Planning- is at the center of self-direction and self-management. All good plans are done in partnership, where the case manager is the person who knows the requirements for a plan and what must be accomplished works in concert with the content experts who are those that know the detail of what the plan needs to say.

  • Referral/linkage- Referral and linkage activities connect a recipient with medical, behavioral, social and other programs, services, and supports to address identified needs and achieve goals specified in the Person Centered Plan.

  • Monitoring/follow-up- Monitoring and follow up includes activities and contacts that are necessary to ensure that the Person Centered Plan is effectively implemented and adequately addresses the needs of the recipient. Monitoring activities may involve the recipient, his or her supports, providers, and others involved in care delivery.
