To refer a consumer to Support Incorporated, please use our Application for Services.  The Application should be completed in its entirety by the referring person and faxed to us at (704) 865-3520.  It is preferred that you phone us at (704) 865-3525 to let us know that an Application has been faxed.  Once we receive the Application, we will start our screening process, which determines if we are able to meet the needs of the referred consumer.  We will keep the referring person/agency informed as to the status of the referral.
