Community Support Counselor

                                                
 

Description of Responsibilities: This is a professional level position within Support, Inc., and will serve a caseload of 15-20 children or adult consumers who are identified as needing Community Support services. This position also allows staff to transition between professional and clinical responsibilities and direct care/service oriented demands inherent in working with this specific population. All persons serving as a Community Support Counselor within Support, Inc. are credentialed professionals (QMHP/QSAP).  These trained professionals have vast clinical knowledge of various human behavior therapy models as well as a through knowledge of the philosophy of various therapeutic approaches. A person in this position is expected to provide indirect and direct care services to our consumers and their families.

 

This will require a Counselor to arrange services of a consumer, link the consumer to community resources, coordinate services between the consumer and other agencies or service areas, assess and identify a consumers need, plan treatment milieu with the consumer, family & other identified agencies as collaborative teams at the table, advocate for the consumer or on behalf of the consumer, and monitor all service provisions that the consumer or family is involved with to ensure consumer safety, best practice models and progression towards self sufficiency. The Counselor is also the responsible person who assists the family and team members identify, implement and progress through the treatment goal plan. This person is also responsible for monitoring all service provisions and informing his/her chain of command and any governmental officials if and when they witness or have knowledge of any abuse, neglect or exploitation of a consumer.

 

Finally, the Community Support Counselor is responsible for billing services to the appropriate fund source via the daily SAL and on-line service note. This paperwork is due within a 24 hour time frame. Minimum billing standards are 27 hrs. weekly minimum.

Essential Job Functions:



Hours of Work: 8:00 am to 5:00 pm, or as assigned by the coordinator. Operation under “whatever it takes” to perform legitimate job duties. Office hours can be flexed out within the work week supervisor/coordinator approval.

Explanation of Any Work Shift Rotation: There is no “shift rotation” as such, however, due to the specialized nature of this position it is expected that a case manager will working the hours needed for overseeing the service provision of all of the consumers on their caseload. Assignments may occur over the weekend and after hours during the week. After hours work will be assigned based on the needs of the program and consumer population. Employee must see to the service needs of the consumers even after regular business hours.

Minimum Education: Minimum requirement of Qualified Professional under 10 NCAC 14V. Preferred educational requirements of a Bachelor’s Degree in the Human Service field (Psychology, Social Work) or other degrees with special consideration given to work experience relevant to this population.

Licenses or Certifications Required: Must be or become NCI certified, receive training in the population served, client rights/confidentiality, agency specific orientation, CPR/First Aid, Preventing Disease Transmission & Seizure Management. A valid driver’s license is also required for this position.  Must complete 20 hours of mandatory training per service definitions.

Special Knowledge, Skills, Physical Requirements or Other Factors: Employee must be in good physical and emotional condition, capable of dealing with consumers who have more than one disability area and who may have violent tendencies. Must attend all assigned trainings required by the Agency, Coordinator or LME. Must be willing and able to adapt to rapid changes and other such factors that are inherent in dealing with these types of services and consumer populations. Must be a team player with a positive spirit and willing to assist and support the team to assure that the Agency operates smoothly. Must have a personal vehicle available for consumer services and other assigned duties. Criminal record check, HCPR and finger printing & a negative drug screening are all prerequisites for employment consideration.

 


 

Associate Professional

                                                           

Primary Purpose of Position:  The primary purpose of this position is to provide Community Support to Mental Health and Substance Abuse consumers or Developmental Therapies or CAP services with  Developmental Disabilities consumers. The person in this position will be an active member in the treatment planning process (i.e. develop goals and step down plans for consumers). This is an off-site job and its purpose is to provide those services in the community to enhance the client’s ability to function and achieve. They include but are not limited to; supportive counseling, psycho educational activities, behavioral interventions, training to caregivers/family support, vocational support, educational support, community integration and any and all areas identified as needed by the client’s individual Service Plan that apply to the identified service.  Coordination of this service will occur under the auspices of the Program Coordinator, Program Supervisor and Treatment Team members. These people will direct actual service delivery. A Qualified Professional, which may include the Responsible Counselor/Therapist, or Program Supervisor who is also Qualified Professional will provide supervision.

 Assist in planning, scheduling and implementing the daily schedule for the clients.

Work to coordinate service delivery with the other staff.

Assist in the development of transition plans when clients move in and out of services.

Develop Therapeutic Interventions to work with client.

Provide any written documentation as required.  This will include treatment and goal plans activity notes for the medical record (e.g., Service notes, SALs, etc.)

Direct Services

      Work with educational, therapy, CS Counselor and habilitation staff in supervising clients and assisting the delivery of Community Support. This includes implementing behavioral interventions, crisis interventions and psycho educational activities.

   Work with clients daily on individual and group basis in reviewing, applying and achieving their goals as indicated on the treatment plans and goals.

     Assist and/or coordinate as assigned in planning and implementing community field trips and other such activities.

      Use of activities as a means to deliver clinically appropriate services.

     Assist other staff in parent/guardian/teacher training that will aid them in dealing more effectively with their children.

      Assist in all service delivery as formally planned with the clinical staff.

Minimum Education, Training and Experience:  Completion of a 4-year Bachelor’s Degree, and has experience with population being served.

Licenses or Certificates Required: Must become NCI certified; receive training in population served, and client rights/confidentiality. Orientation to; Support, Inc., Recovery Model and Systems of Care is also required. The employee must become privileged in delivery of Community Based Services. CPR/First Aid, Medication Administration and Preventive Disease will be required.  Valid Driver’s License is also required. An employee must demonstrate having the core skills to deliver Community Support prior to employment.



 


Paraprofessional
